1.效期6個月以上之護照

Passport valid for at least 6 months

2.簽證申請表（請上領務局網頁填寫並下載列印具條碼之申請表。另申請表必須由申請人親自簽名以確認申報內容屬實。）

Visa application form (Please fill out and download the form with a barcode from the consular affairs website. Additionally, the applicant must personally sign the application form to confirm the authenticity of the information provided.)

3.申請表上必須黏貼兩吋照片兩張，照片必須是最近6個月以內所拍攝、背景為白色的彩色照

Two recent two-inch color photos with a white background must be affixed to the application form. These photos should have been taken within the last 6 months.

4.備妥與來臺目的相關之證明文件。

Prepare relevant documents supporting the purpose of the visit to Taiwan.
